MONTOUR FALLS, NY –Schuyler Hospital has announced that their 2010-2012 Community Service Plan is now available on-line via their website at www.schuylerhospital.orgThe report, completed in the fall of 2009, was the result of working closely with Schuyler County Public Health, as well as a number of community-based organizations and focus groups, and reviewing a variety of studies and data.The resulting report focuses on four prevention agenda priorities. Access to Healthcare, Mental Health & Substance Abuse, Healthy Moms & Children, and Physical Activity & Nutrition. Work groups are currently meeting to develop plans, programs and outreach to affect sustainable change in each of the identified priority areas over the next 3-year period.The document, prepared by Schuyler Hospital, was submitted to the State of New York Department of Health pursuant to Public Health Law 2803-1.
