The Canandaigua VA Medical Center (VAMC) and Rochester VA Outpatient Clinic (ROPC) are currently enrolling Veterans into the Telehealth Program. Veterans enrolled in this program will receive a home monitoring device which, depending on the veterans needs, provides daily monitoring by a registered nurse for blood pressures, blood glucoses, weight and other significant information.
